Work Violations Monitoring Program — 2

My job is monitoring safety violations based on company standard, And what I do is using excel to collect violations,
I wanna build simple program with data base of violations types and amount and I want to attach documents and pictures,

There is standard form for violations I want a choice that work as follow: after registring a violation on the system I want it to be able to be printed on the form directly,
