Website for teacher appraisal

I am looking to get a website built for the purpose of teacher appraisal. The website will need to have online evaluation forms, be user-friendly, and be built using any language/platform that works best for the project. With this website, I will be able to track the performance of teachers and have a direct line of communication between administrators and teachers. The website must be easy to use for my teachers and administrators and have the flexibility to accommodate necessary changes and additions in the future.
