Python automation to fill and submit form

Hello, I need help with python automating a task in a website. Basically, a form needs to be automatically populated and submitted. See the vdo link below for more details.

Please tell me quickly how much would you charge, how many days you need to do job. Your delivery will be a python script will necessary files, along with a step by step instruction document on how to make it work, preferably from a windows machine.
