PHP Json call and display data

Hey this should be pretty easy if you know php/html,

I have a site template that i’ve edited for my needs, I just need the user to input their iPhone IMEI into the input field and when the click the “check now” button it should use that IMEI to get a JSON response from an api I will provide.

Then once the JSON data is pulled it should store > phone’s model number, phone version, & unhide a div and display those variables (I also need it to check if the device is MEID which can be displayed in a format of “MEID: True/False” and this can be found by the model number so it will need to check some sort of dictionary to see if it’s MEID)
