PDF Form Specialist / PDF Specialist

Looking for someone who specializes in PDF forms!

What we need which is different from standard – we want to create a PDF form which allows user to upload pictures into it, whichout messing up the page layout. We have the ability to define the picture size and limit the upload options.

attached is an example of what it will look like finished – but the images are what I want the user to select and upload as part of the form

Need a very urgent turnaround – if all goes well, there will be many projects like this going forward.
