modify solidworks file as shown in pictures

Hello,
I would like a solid bid (do not try and negotiate afterwards) for modifying the solidworks files as shown in pictures 1-3. Also, I need a template drawn as shown on picture 4.
Please bid a final number and not try and negotiate later.
