Leave Request App in Power Apps

Leave Request App Scope.

The app will require the following functionality. like the template on offer in power apps.
Ability for employees to submit a leave request with following information.
type of leave e.g., Annual Leave, Personal Leave, Sick leave.
First Date of Leave – date picker
Last Day of Leave – date picker
First Day Back at Work – date picker
Annual Leave Days – number dropdown box
Personal Leave Days – number dropdown box
Sick Leave Days – number dropdown box
Signature of Employee
Manager Name (user office365 manager field for this information.
Submit request.

Approval request then gets submitted to manager (using the manger field in office365 for the managers details and email address)
Manager will get an email of the requested leave.

Managers section that lists pending and approved leave for the staff they look after.
Ability for company gm/hr to view all pending and approved leave.

Approved Leave to get submitted to hr email for a record.
Ability to have all approved leave displayed on a webpage (via sharepoint?)
company branding to be included.
