Latex template!.

HI everyone!
i need a complete latex template like a custom writter exam, with response layer and other stuff. I can send you my very basic template to take it like a guide. I need the template, not the content.
1.- On the header must be some charecteristic of the institution (all pages).
2.- The pdf (that i can send you) show all the exams sitcked, i need reset page number for each exam and set the same header( title, owner etc).
3.- Each question must not be breaked by the pagebreak, question and alternatives must be always together.
4.- On the “answer page” and “right answer page” if the quantity is big, latex will deploy it in two columns. Always up to down direction.
