I need some forms built.

I need a number of forms built. They will be between 1/2 a page to 1 1/2 pages long. I need the price you would charge for each form. I’m a sales person and I don’t know anything a bout programming. Each of the forms will be connected to a Word document. My people will do that. I will up load the first two Forms I need
