GOOGLE SHEET BASED APP REQUIRED FOR MY EMPLOYEES TO APPLY FOR LEAVES

I have a system in my store to grant my staff either a PL or a UL – that is either a paid leave or Unpaid leave. the condition is that not more than 3 employees should be able to apply for a PL per day and max PLs allowed per employee per year is 25. Now I need a simple Sheets based app that my employees can download and apply for a leave & such data needs to be visible to me and them too..
