Forminator Plugin / WordPress

I need a forminator expert, who can build me a formular. Some check boxes, some text fields, some accordions (!) for some additonal text. Also an option to send attachments. All data will be send (send button) to a certain email adress. Draft of the formular (pdf) is already available (see attachment). Design like: Part of the website is already online…ask for password (www.velo-check.de), in case you are interested.
