Excel Sheet Template

We need someone to use our ‘appraisal form for watches’ and turn it into an ‘excel template for watch identification cards’. We currently use FileMaker to design the identification cards but it is too long of a process to transfer info from the excel sheet into FileMaker one by one. Ideally we need the excel template for the card to autofill/populate from the excel appraisal form. This info will be the description, photo, report number, and date
