Ethernet to SFP Tranceiver media conversion

I want a small board designed which will operate at 5-12 V, in the -40 to 65C temperature environment and which does media conversion from 10/100/1000 Ethernet to SFP Transceiver. The ethernet and transceiver cage housing the transceiver should be on the opposite ends of the board exposed to the customer.
