Goal is to create a database that allows members access to their own personal information (ideally w/ a login, but if not, some combination of a DOB and Roll Number….

Right now there is no database that centralizes our enrollment and so it is causing a lot of issues. This is a database that is created outside of the organization so I a having to rely on members to fill in the info themselves

The attached spreadsheet has all of the information that I have and I am going to be requesting members each fill out information. I don’t have contact info, but will be blasted on facebook groups.

Create a portal that allows for these fields to be updated via a form of some kind and to have each member (each row is a member) access to their own information. Each row in the spreadsheet is a person:

Personal View:
Roll Number
Kiowa District
Current Status
First Name
Middle Name
Last Name
Suffix
Date of Birth
Unique ID – This can be anything – I’m open to suggestions but thinking it is a combination of their date of birth and roll number or something
Email Address
Phone Number
Physical Address
City
State
Zip Code
Registered Voter
Degree of Indian Blood
Other Indian Blood

Family Members:
Now every enrolled member is a Row on the spreadsheet and I am assuming that virtually all Fathers and Mothers are already on here.– Can these fields choose by typing in their name but it would select from the rows of candidate populated dynamically. They need at least one mother or father with Kiowa bloodline, so if you can set it up so at least 1 is required but 2 if both are Kiowa. If they can’t find them in the database, would it be possible to put a roll number in lieu of their info just in case?

If that is too complicated, just make separate fields Mom/Dad First and Last name

Children under 18 can be enrolled, and obviously not everyone has children, so if you could just have the children populate if they answer yes to kids or something;

Current Status (same as other one cu they are all records)

Things In Red Are Required for Every New Update
Should Have some Mechanism for Checking for Duplicates
Nothing Should Post without getting approved by me, or an admin
Should be able to go in and make edits pretty easily

Then if there was an area for some basic reporting – awesome.
