Build a Software for my Covid Test center

I need a Software for my Corona Test Center. Customer can register via form
We see customer in admin panel and confirm him
For confirmed customers we click on the customer and enter the result
A PDF is created with the data with a digital signature
The customer receives the PDF by email

Procedure:
Dropdown where you can choose service E.g. Antigen test
Select appointment Form
First name
Surname
Phone
Email
ZIP City
Street House number
Date of birth (later also for decryption test)
Checkbox: Data protection
Checkbox: result to email
The customer then receives an email for confirmation

Test station side:
Scan the customer’s QR code
Admin Panel: We see the tests and customers
Can enter the result there (positive, negative, no result)
Send result by email
