Basic App Bluetooth from RFID stick reader to Mobile App

I require a App simular to Tagmax. I have a RFID stick reader which is Bluetooth. Once I scan the microchip or RFID ear tag, it will transmit the the serial number to the App into the field called EID (Electronic ID). Below this field, I must be able to add the Ear Tag number of the animal and add several fields below this for example, Weight, Health, etc.
Have a look at Tagmax App and you will see what I require. The information must be saved and be able to export to a spreedsheet.
