azure form recognizer automation

Hello,
I am working on a process automation using Uipath. one of the section in the project requires extract data from a form which some times fill by hand. I am using Azure from recognizer service to train a model and call the trained model to Uipath. I have trained the model using the online “Form Recognizer Sample Labeling Tool” (https://fott-preview.azurewebsites.net). I have the trained model ID, however I need someone who knows how to call the model ID in Uipath, then using the trained model to read the forms and extract data in Uipath.
