plugin reading and comprehension speed test

I need a plugin reading and comprehension speed test, and it should perform the following function:

1st step, a section where to put the text to be read by the user
2nd step, from 5 to 10 multiple choice questions, and with the option to place value or weight on the answer
3rd step, filling out the contact form (name, surname, email, phone, city)

After completing the 3 steps, the plugin must send an email to the user and admin with the data:
– time it took the user to read (1st step)
– result of the answered questions (2nd step)
– initial greeting with your name (3rd step)

Necesito un plugin de examen de velocidad de lectura y comprension, debe cumplir con las siguientes funciones:

1er paso, una seccion donde colocar el texo a leer por el usuario
2do paso, de 5 a 10 preguntas de opcion multiple, y con la opcion de colocar peso o puntaje a las respuestas
3er paso, llenado del formulario de contacto (nombre, apellido, email, telefono, ciudad)

luego de completado los 3 pasos, el plugin debe de enviar un correo al usuario y administrador con la siguiente informacion:
– tiempo que le tomo en leer (1er paso)
– resultado de las preguntas (2do paso)
– saludos inicial con su nombre (3er paso)
