Youth-transitioning-out-of-care need access to resources and support to help with their journey. They are looking to stay connected with the programs and services available to them– and need an easy portal to access everything they need in one place.

A very important aspect of this project is the gathering the necessary resources and information– followed by laying it all out in a user focused way. The keys to a successful user experience are thoughtfully mapped-out user journeys that align with overall website goals along with ongoing stakeholder collaboration.
