Public Records Request program

I’m interested in obtaining a quote/estimate for creating a Public Records Request database/form(?) that will be used as a public service for those seeking to do FOIA/Public Records requests. There will be no names, addresses, email, or personal info obtained from the users, only the pertinent info relating to the request. The model I’m looking at would be similar to the one found in the link below. It doesn’t have to be quite as detailed, actually the more simple the better.

This project will reside on a Linux WordPress site. The form request will be created using Fluent Forms Pro. If you are familiar with these two programs, I would prefer to speak with you.
