create an access report for book template

I need to recreate a report template in access like the attachment. The user will chose an image file from a form and fill some fields
I don’t need the form i only need the report designed like so
I really wish to let the user being able to select color of the under part so it has to be some objects not only an image
Dimensions are 450×297
