Excel opening configuration form

I’m trying to create a pop up form for use within Excel that uses ~30 drop down menus, ~15 check boxes, and ~5 text boxes. Every time there is a submission it generates a table with up to 15 rows and 4 columns. Number of rows is determined by which boxes are checked. Each subsequent submission generates below the previous.
