Form with calculated filds for WordPress + Print PDF

I need to implement a custom form with calculated fields for wordpress. I upload the specific conditions of the single fields. The user can print the final form in a PDF file with the site logo on top, all the filed name and input data and a nice graphic look.

The form must be implementable in Elementor for easy maintenance.
