Submit Form

Need to Create a submit form (template)
– Responsive on all mobile devices and be able to connect it to a database
with the exact measurement (plz check a1.a2)
– The submit form should look also centre on desktop (same designee as mobile)
– all png file uploaded need to be used in that design
– i need s very clean code and comments over cuss rules and the php file connection to the database
– also if submited the form with empty fields or wrong email format (example@domain) an error pops out inside those fields.
– also need to include time stamp [ $timestamp = date(‘Y-m-d H:i:s’); ] <<< with this format
