Build me basic website/portal for COVID Testing that generates COVID QR code with test result

If you have built a similar website/app then I would be interested in simply purchasing your existing project

This portal/website will be for providing 10 minute rapid covid tests to customers in person that customers can use to gain entry into public spaces like bars and shopping malls. The customer needs a scannable QR code that shows their basic information and another field showing negative test.
