php and Javescript programmer needed

Hi I have a number of project for $30 Each project
The First project will be for a maximum budget of 30 just to see if awarded freelancer is reliable and can finish each project completed on time. How the first project I would like to start is: (I can create milestones for $15 and realise after the project will be completed. Plus the second $15 milestone will be created and realise imidietly after the 1st project will be complete. )
The first project will have to use PHP programming to create a Form that after submit all data will be saved/insert to a database table. I will provide the correct credential to chosen awarded freelance and you will have to send me the file that I can test with hosting server databese.
Some JavaScript programming for the Form may be needed.
Please only serious bids, not over $15 initials milestones requirements
